Connect to the ICN

Connecting to the ICN is easy. Download the application form and mail or fax it to us. All forms are in Adobe Acrobat format.

 For new ICN applicants:

ICN Master Service Agreement
This is the agreement form to participate in the ICN. The ICN Master Service Agreement is required of all fiscal organizations whether connecting directly or indirectly to the ICN.

The ICN Service Order Form will be completed by your Regional Technology Center and sent to constituents who are directly connected to the ICN as a follow up to their Master Service Agreement. The form is provided here for information purposes and does not need to be filled out or returned.

Acceptable Use Policy

Additional IP Addresses Request
If you are a current Participant and require additional IP addresses because of growth in computers, network arrangement, or Internet services, please fill out and return this form.

E-rate LOA
Form needed from all e-rate eligible entities acknowledging that the ICN is filing for reimbursement on the constituents behalf.

ICN Management Release Agreement
This waiver form is needed if you want to maintain and troubleshoot your own router.

Last Mile Service Provider
This is a list of last mile service provider options for connecting to the ICN.

Letter of Agency
This document is needed in certain situations and allows the ICN to order circuits on behalf of the customer.

Service Level Agreement
This document describes the services provided by CMS for which this SLA applies, details  the support request procedure to be used in th event of problems once the service has been commissioned, and identifies level of credits for Service Disruptions.
